About the Role

We are seeking a skilled and compassionate Rehabilitation Support Worker to join our integrated Health and Social Care community rehabilitation service in Eastbourne. As a key member of our team, you will play a vital role in supporting individuals in their own homes, promoting independence, and enhancing their quality of life.

Key Responsibilities
  • Visit clients in their own homes, providing person-centered support and care
  • Liaise with clients in acute and intermediate care settings to ensure seamless transitions
  • Work collaboratively with the multidisciplinary team to develop and implement personalized care plans
  • Contribute to the assessment and treatment of clients, promoting independence and quality of life
Requirements
  • GCSE grade 4 or GCE C and above in relevant subjects
  • Level 2 functional skills or health care support worker apprenticeship/NVQ level 2 in relevant subject
  • Level 3 apprenticeship or NVQ level 3 in relevant subject or equivalent level of knowledge and experience
  • Experience in health or social care settings, demonstrating the values of the NHS and commitment to personal and professional development
