About the Role

We are seeking a skilled and compassionate Rehabilitation Support Worker to join our integrated Health and Social Care community rehabilitation service in Eastbourne, covering the Eastbourne Hailsham & Seaford area.

Job Summary

The successful candidate will be part of a friendly, supportive, and professional team that provides a 7-day service using an inter-professional model of working. The role involves key working non-complex clients, providing rehabilitation to people with various conditions, and responding to urgent referrals to support clients to stay in their own home.

Main Responsibilities

  • Provide rehabilitation support to clients with a variety of conditions, promoting independence and quality of life.
  • Key work non-complex clients, providing ongoing support and guidance.
  • Respond to urgent referrals, supporting clients to stay in their own home.
  • Work collaboratively with the multidisciplinary team to achieve client-centered outcomes.

Requirements

  • GCSE grade 4 or GCE C and above.
  • Level 2 functional skills or healthcare support worker apprenticeship/NVQ level 2 in a relevant subject.
  • Level 3 apprenticeship or NVQ level 3 in a relevant subject or equivalent level of knowledge and experience.
  • Experience in health or social care settings, such as employment, work experience, volunteering, or placements.
  • Demonstrate the values of the NHS and commitment to personal and professional development.

Working with Us

The post holder will be required to work flexibly and visit clients in their own homes, liaising with clients in acute and intermediate care settings if necessary prior to discharge. The ability to travel around the county in a timely fashion is essential.
