About the Role

We are seeking a skilled and compassionate Rehabilitation Support Worker to join our integrated Health and Social Care community rehabilitation service in Eastbourne, covering the Eastbourne Hailsham & Seaford area.

Job Summary

The successful candidate will be part of a friendly, supportive, and professional team that provides a 7-day service using an inter-professional model of working. The role involves key working non-complex clients, providing rehabilitation to people with various conditions, and responding to urgent referrals to support clients to stay in their own home.

Main Responsibilities

The Rehabilitation Support Worker will be required to work flexibly, visiting people in their own homes and liaising with clients in acute and intermediate care settings if necessary prior to discharge. The ability to travel around the county in a timely fashion is essential.

Person Specification

The ideal candidate will have:

  • GCSE grade 4 or GCE C and above
  • Level 2 functional skills or healthcare support worker apprenticeship/NVQ level 2 in a relevant subject
  • Level 3 apprenticeship or NVQ level 3 in a relevant subject or equivalent level of knowledge and experience
  • Experience in health or social care settings, such as employment, work experience, volunteering, or placements
  • Demonstrating respect, kindness, and empathy at all times
  • Understanding of role boundaries and the importance of supervision and carrying out appropriately delegated tasks
  • Able to understand and follow procedures, protocols, and plans
