Community Forensic Mental Health Nurse Specialist

We are seeking an experienced and skilled Community Forensic Mental Health Nurse to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our Forensic Intensive Recovery Support Team (FIRST), you will play a vital role in supporting individuals with complex mental health needs and histories of offending or risk behaviors.
About the RoleThe successful candidate will be responsible for working collaboratively with service users, carers, and other healthcare professionals to provide high-quality care and support. This will involve assessing, planning, implementing, and evaluating individualized care plans, as well as promoting recovery and transition from inpatient settings to community-based care.
You will also be expected to participate in out-of-hours on-call rotas to support crisis responses and provide consultation, support, and clinical supervision to other healthcare professionals as needed.
Key Responsibilities- Manage a complex caseload of service users with complex mental health needs and histories of offending or risk behaviors.
- Assess, plan, implement, and evaluate individualized care plans in collaboration with service users, carers, and other healthcare professionals.
- Provide high-quality care and support to service users, including therapeutic interventions and risk assessments.
- Participate in out-of-hours on-call rotas to support crisis responses.
- Provide consultation, support, and clinical supervision to other healthcare professionals as needed.
We are looking for an experienced and skilled Community Forensic Mental Health Nurse who possesses the following qualifications and skills:
- Registered mental health nurse on the NMC register.
- Post-registration experience in forensic or secure environments.
- Experience of working with individuals with complex mental health needs and histories of offending or risk behaviors.
- Ability to work collaboratively with service users, carers, and other healthcare professionals.
- Strong assessment and planning skills, with the ability to develop and implement individualized care plans.
- Excellent communication and interpersonal skills.
