About the Role

We are seeking an experienced and enthusiastic nurse to join our expanding team and work within the Forensic Intensive Recovery and Support Team (FIRST).

FIRST work closely with other forensic services in the West Midlands, as part of the REACH OUT partnership, a new provider collaborative.

The post holder will be part of a multi-disciplinary service supporting transition of patient care from secure settings into the community.

Main Duties

  • Work collaboratively with service users, carers, family and friends and housing providers providing support, and spending therapeutic time with an allocated group of service users with complex mental health and social needs.
  • Responsible for care coordination of an identified caseload, promoting service-users recovery and transition process from an inpatient setting.
  • Assess, plan, implement and evaluate individualised care/recovery plans, using a variety of appropriate clinical and risk assessments.

Person Specification

  • Registered mental health Nurse on NMC Register.
  • Post-registration experience, evidence of formal courses and training at diploma/degree level.
  • Registered Mentor
  • Evidence of extensive post registration experience with working with adults experiencing mental health related difficulties
