We are currently seeking a skilled Registered General Nurse to join our Discharge Lounge team at either Darlington Memorial Hospital or University Hospital of North Durham.

About the Role

The successful candidate will be responsible for supervising Health Care Assistants, liaising with the Discharge management team/ward coordinators and relatives, and ensuring patient care during their time in the discharge lounge. This role requires effective communication skills to create and build a good network of relationships with all wards and departments.

The ideal candidate will have a strong interest in discharge planning and be flexible regarding shift patterns and sites in accordance with service needs. They will also possess excellent organizational skills, with the ability to prioritize tasks effectively and work well under pressure.

Key Responsibilities
  1. To assess patients' needs and develop, implement, and evaluate programs of care.
  2. To organize the delivery of nursing care, deploying staff effectively.
  3. To communicate changes to patient information and treatment plans, both verbally and in writing.
  4. To adhere to NMC standards of record keeping.
  5. To assist the Ward Sister in setting, monitoring, and maintaining standards of care.
  6. To participate in nursing audit and service improvement activities.
Requirements
  • NMC Registered.
  • Flexibility regarding shift patterns and sites in accordance with service needs.
