Clinical Care Coordinator

We seek a highly experienced Registered Nurse (RGN/RMN/RNLD) to join our team in a complex care home environment. As a key member of our multi-disciplinary team, you will contribute to delivering exceptional care and support to our residents.
Your primary responsibilities will involve:
- Developing and implementing comprehensive care plans that address the unique needs of each resident, fostering independence and self-esteem.
- Participating in multidisciplinary meetings to discuss resident care and collaborate with other healthcare professionals on treatment plans.
About You
To succeed in this role, you must have:
- A valid NMC registration and in-depth knowledge of up-to-date clinical practices, particularly in dementia care.
- Proven experience in producing high-quality care plans and conducting thorough risk assessments, showcasing your analytical skills and ability to work under pressure.
Salary and Benefits Package
