About the Role

We are seeking an experienced and dedicated Professional Care Coordinator to join our Adult Learning Disabilities and Physical Disabilities Team in Darlington.

As a key member of our team, you will work closely with adults who have learning and physical disabilities, helping them achieve greater independence, well-being, and access to tailored support services.

Main Responsibilities:
  • Conduct comprehensive assessments to determine the needs of adults with learning and physical disabilities, developing personalized care plans that promote independence and quality of life.
  • Collaborate with health professionals, housing services, and other agencies to provide a coordinated approach to care.
  • Advocate for individuals to ensure their rights are respected, and they have access to necessary services and resources.
  • Ensure compliance with relevant legislation, including the Care Act and Mental Capacity Act, adhering to safeguarding procedures.
  • Support adults in transitioning to or maintaining independent living arrangements, making referrals to suitable community resources.
Requirements:
  • Qualified Social Worker with valid Social Work England registration.
  • Experience working with adults with learning disabilities and/or physical disabilities.
  • Strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding practices.
  • Ability to work collaboratively within a multidisciplinary team.
  • Commitment to promoting equality, diversity, and inclusion in all aspects of work.
About the Salary

The estimated salary for this role is £35,000 - £45,000 per annum, depending on experience.
