About the Role

We are seeking a skilled and compassionate Registered Mental Health Nurse to join our Community Transformation programme as a Senior Community Mental Health Practitioner. This is an exciting opportunity to work with a multidisciplinary team, providing recovery-focused treatment and support to individuals with mental health disorders.

Key Responsibilities

  • Work as part of an established multi-disciplinary team, providing holistic care to individuals experiencing mental health issues and supporting their recovery journey.
  • Act as a care coordinator/lead professional for a dedicated caseload of service users.
  • Support carers and system partners.
  • Undertake mental health nursing needs assessments/care act assessments, as required.
  • Complete social circumstance reports and attend mental health review tribunals.
  • Administer prescribed depot injections/medication monitoring and clozapine clinic, as required.
