About the Role

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our established Multidisciplinary team within the Community Mental Health setting. As a key member of our team, you will provide holistic care to individuals experiencing mental health issues and support their recovery journey.

Main Responsibilities
  • Work collaboratively with a Multidisciplinary Team to provide comprehensive assessments and formulate personalized care plans that reflect the needs of service users and their families.
  • Act as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, ensuring timely and effective interventions.
  • Undertake Mental Health Nursing Needs Assessments/Care Act Assessments, as required, to inform care planning and decision-making.
  • Administer prescribed depot injections/medication monitoring and Clozapine Clinic, as required, to ensure safe and effective medication management.
  • Conduct risk assessments of individuals and their environments to identify potential hazards and develop strategies for mitigation.
