About the Role

We are seeking a skilled and compassionate Registered Mental Health Nurse to join our Community Transformation programme. As a key member of our Multidisciplinary team, you will provide holistic care to individuals experiencing mental health issues and support their recovery journey.

Key Responsibilities

  • Working as part of an established Multi-Disciplinary Team within the Community Mental Health setting, providing care to those who are experiencing mental health issues and supporting their recovery journey.
  • Acting as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users.
  • Supporting carers and system partners.
  • Undertaking Mental Health Nursing Needs Assessments/Care Act Assessments, as required.
  • Completing Social Circumstance reports and attendance at Mental Health Review Tribunals.
  • Administration of prescribed depot injections/medication monitoring and Clozapine Clinic (as required).
