Job Summary

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our established Multidisciplinary team within our Community Transformation programme. As a key member of our team, you will play a vital role in providing recovery-focused treatment and support to individuals with mental health disorders, empowering them to lead fulfilling lives.

Main Responsibilities
  • Work collaboratively with a Multidisciplinary Team to provide holistic care to individuals experiencing mental health issues and support their recovery journey.
  • Act as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, ensuring their needs are met and their care plans are tailored to their individual requirements.
  • Support carers and system partners, fostering strong relationships and promoting effective communication.
  • Undertake Mental Health Nursing Needs Assessments/Care Act Assessments, as required, to inform care planning and ensure the best possible outcomes for service users.
  • Complete Social Circumstance reports and attend Mental Health Review Tribunals, providing expert testimony and advocating for service users' needs.
  • Administer prescribed depot injections/medication monitoring and Clozapine Clinic, as required, ensuring the safe and effective management of medication.
  • Conduct comprehensive assessments, formulating personalised care plans that reflect the needs of service users, their families, and wider support networks.
  • Provide psycho-social interventions, education, and support to develop service users' and carers' understanding of mental illness, aiming to prevent relapse and admission.
