About the Role

Priory Medical Group is seeking a skilled Digital Care Coordinator to join our team. As a Digital Care Coordinator, you will play a vital role in ensuring that our patients receive seamless and coordinated care.

Key Responsibilities

  • Provide coordination and navigation for patients through the healthcare system
  • Coordinate clinic sessions to support complex and vulnerable patients or those who are non-digitally enabled to access care
  • Undertake outbound calls to identified non-digitally enabled patients who need a phone call or letter invite for appointments to ensure digital inclusivity and support their long-term condition management
  • Book patient appointments into services provided by healthcare professionals across PMG
  • Support patients in preparing for follow-up with provision of information on what they should expect when they attend for their appointment
  • Document submitted data against protocols, file in the patient record, and appropriately signpost for follow-up care
  • Work with wider PMG teams to continually improve and develop the non-digital care coordination offer to patients
