About the Role

Priory Medical Group is seeking a skilled Digital and Care Continuity Coordinator to join our dynamic team. As a key member of our digital team, you will play a vital role in ensuring our non-digital patients are not excluded from care coordination.

Main Responsibilities

As a Digital and Care Continuity Coordinator, you will be responsible for:

  • Coordinating patient care through non-digital communication tools
  • Working with the rota, digital, and patient central team to confirm clinic capacity for urgent care
  • Maintaining our digital systems and tools
  • Providing support to patients in preparing for follow-up appointments
  • Documenting patient data and submitting it against protocols

Priory Medical Group is a large eight-site GP practice and single Primary Care Network based in York. We pride ourselves on being innovative and forward-thinking, using digital tools to support our patients and drive the delivery of accessible and responsive care.
