Care Coordinator

Job Title: Domicillary Care Coordinator
About the Role:
We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home Instead. As a Care Coordinator, you will be responsible for assessing the care needs of our clients, creating personalized care plans, and ensuring that our care staff are equipped to provide the highest level of care.
Key Responsibilities:
- Meet with clients to assess their care needs and preferences
- Develop and implement care plans that meet the unique needs of each client
- Support care staff through observations, support visits, and supervisions
- Ensure that care staff are equipped to provide the highest level of care
- Coordinate care schedules to ensure that clients receive the care they need
- Communicate effectively with clients, care staff, and other stakeholders
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements:
- Full British driving licence required
- Computer literacy
- NVQ Level 3 in Health and Social Care
- Experience of working in a scheduling role within a home care or other relevant environment
- Highly resilient and positive with excellent customer service and communication skills
- Excellent organisation and prioritisation skills with the ability to work calmly and accurately under pressure
- Team player with strong interpersonal skills with the ability to build rapport quickly
- Excellent attention to detail with the ability to multi-task
