Job Summary:

The NHS is seeking a skilled Mental Health Care Coordinator to join their team in West London. This is a unique opportunity to work with a dedicated team in a Nurse-led Clozapine clinic, providing holistic care to patients with complex needs.

About the Role:

This challenging and rewarding role requires an individual with excellent communication skills, empathy, and a strong understanding of mental health needs. The successful candidate will be responsible for conducting physical health screenings and assessments for Clozapine patients, as well as assisting with blood sampling and participating in comprehensive care coordination.

Key Responsibilities:

  • Conducting physical health screenings and assessments for Clozapine patients
  • Assisting with blood sampling and phlebotomy support
  • Collaborating with clinicians to design and deliver collaborative care plans
  • Providing emotional support and enabling access to services for patients
  • Working closely with the Nurse-led clinic team to achieve key performance indicators

Requirements:

  • Degree in Health and Social Care or related field
  • Experience working in a Nurse-led Clinic environment
  • Phlebotomist training and experience
  • Able to demonstrate sound knowledge of the Community Mental Health Framework
  • Excellent communication and interpersonal skills

Salary: £30,000 - £35,000 per annum
