We are seeking a skilled Community Care Specialist to join our team at Remedy Social Work.
About the RoleThe successful candidate will be responsible for providing social work services to patients and their families in the community, working autonomously as part of a multidisciplinary team. This includes performing Care Act assessments, Mental Capacity Assessments, and Safeguarding enquiries to ensure residents receive support under the Care Act.
Key Responsibilities- Identify and assess risks to vulnerable residents and families.
- Coordinate community care services with statutory and voluntary organizations.
A competitive salary of $60,000 - $80,000 per annum, depending on experience.
