About the Role

We are seeking a highly skilled and dedicated Holistic Care Coordinator to join our team in a Nurse-led Clozapine clinic. This is an exciting opportunity to work with patients who require holistic care and support.

The successful candidate will play a key role in providing physical health screenings, phlebotomy support, and comprehensive care coordination. They will also collaborate with experienced clinicians to design and review individualized care plans tailored to each patient's needs.

This is a challenging yet rewarding role that requires empathy, excellent communication skills, and the ability to work effectively within a multidisciplinary team. Estimated Salary: £32,000 - £40,000 per annum (dependent on experience).

Key Responsibilities:

  • Provide physical health screenings and assessments for Clozapine patients
  • Assist with blood sampling as needed to ensure timely monitoring and care
  • Collaborate with clinicians to design and review individualized care plans
  • Participate in comprehensive care coordination to ensure seamless and effective care
  • Travel to various locations throughout Hounslow, Ealing, and Hammersmith and Fulham
