Job Overview

Barts Health NHS Trust is seeking an experienced individual to fill the role of Senior Patient Flow Coordinator. As part of the Complex Discharge Team, you will be responsible for ensuring the smooth and timely discharge of patients from hospital.

Main Responsibilities

  1. Establish and maintain effective electronic databases and filing systems for senior management.
  2. Prepare papers for archiving or destruction in accordance with the Trust's Records Management policy.
  3. Collate information from staff members to produce reports with tables and graphs for distribution.
  4. Receive and prioritize correspondence, ensuring efficient and effective handling.
  5. Arrange meetings for senior management and type documents to a high standard.
  6. Handle confidential documentation and personnel matters discreetly.

Person Specification

  • Essential: A-Level, evidence of continued professional development, significant experience in acute/tertiary hospital settings, and ability to demonstrate expertise at senior level of Admin/PFC.
  • Desirable: experience of managing staff, recent operational management experience, and involvement in improvement programmes/project work.

Knowledge and Skills

  • Essential: knowledge of discharge planning processes, legislation, and policies, as well as good knowledge of Excel sheets and data analysis.
  • Desirable: flexibility to adapt skill base, competent in development and delivery of programmes of care.

Working Environment

The role will be based in the CDT office, where you will work closely with ward staff, other professionals, and agencies to ensure accurate information and up-to-date records. You will also be responsible for undertaking key tasks during admission and discharge to ensure decisions are put into action and followed up.
