Patient Flow Coordinator

About the Role
We are seeking an enthusiastic and motivated individual to join our Complex Discharge Team at Barts Health NHS Trust. The successful candidate will have excellent organizational and interpersonal skills, with a commitment to delivering high-quality patient care.
Key Responsibilities
- Support the Complex Discharge Team in facilitating patient flow and effective discharge planning.
- Prepare regular and detailed reports on medically optimised and delayed discharges for senior managers and project board meetings.
- Work closely with the Complex Discharge Coordinators and Multi-Disciplinary Team (MDT) to facilitate safe discharges.
- Undertake urgent discharge activities as required.
