We are seeking a skilled Mental Health Practitioner to join our Psychosis Service team in Brent, working with individuals experiencing their first episode of psychosis.
Job SummaryThis is an exciting opportunity for a qualified RMN, Occupational Therapist, or Social Worker to develop their skills and expertise in mental health care. As a Mental Health Practitioner, you will work collaboratively with a multidisciplinary team to provide comprehensive assessments, care coordination, and support to patients and their families.
Key Responsibilities- Assessment and Care Planning: Conduct initial and comprehensive assessments of health and social care needs, developing personalized care plans that meet the identified needs of patients.
- Care Coordination: Lead on the overall care of an assigned caseload, working closely with the multidisciplinary team to ensure seamless delivery of mental health care pathways.
- Multidisciplinary Team Activities: Participate in MDT activities, including duty rotas and clinical meetings, to ensure a shared approach to care, treatment, and risk management.
- Medication Management: Provide advice on medication management, promoting concordance and monitoring side effects in accordance with professional guidelines.
- Physical and Mental Health Reviews: Facilitate undertaking of physical and mental health reviews to ensure holistic care.
- Carer Support: Provide information, advice, and assessment to carers as required, promoting family involvement in patient care.
