About the Role:

We are seeking a skilled Social Care Practitioner to join our team in promoting a whole service approach, working together with internal and external colleagues to deliver high-quality care and support to citizens in need.

Key Responsibilities:

  • Undertake support conversations, strength-based Care Act assessments, Carers Assessments, and reviews with citizens, ensuring they access appropriate support in line with departmental policies and procedures.
  • Conduct Mental Capacity Assessments and Best Interests within the statutory requirements of the Mental Capacity Act 2005, with support from qualified Social Workers as needed.
  • Ensure citizens receive the right professional expertise for their presenting needs, preventing or delaying the need for care and support through maximising independence, autonomy, community engagement, managing or enabling risk, and health promotion.
