Our team is committed to delivering high-quality care and reducing health inequalities across our patient population. We are redesigning our community mental health services to provide a one-team approach, working together with Primary Care, voluntary services, and AWP to provide a seamless care experience for our patients.
As a Mental Health Practitioner, you will work closely with our communities, local authorities, and patients to address issues such as housing, debt, carer fatigue, and addictions alongside mental health problems. You will be part of a multidisciplinary team, working with care navigators, peer support workers, and other professionals to deliver care from GP surgeries.
Key responsibilities include:
- Contribute to the development of the primary care mental health service model, including care pathways for integrated physical and mental health care.
- Be the point of contact for patients directed from primary care, promoting early assessment and treatment, and ensuring robust relationships with community-based services.
- Work closely with the primary care team, including social prescribers and recovery navigators, to support adults with mental health needs.
- Connect people to community and voluntary sector support, working alongside Social Prescribers.
- Facilitate mental health and strengths-based assessments in Primary Care and support the Primary Care Team in accessing appropriate services.
- Build and facilitate closer links between Primary Care Networks and Community Mental Health Teams, Social Care teams, community services, and voluntary sector organizations.
- Provide targeted learning opportunities for primary care staff to develop their knowledge and capabilities in mental health care.
- Foster strong collaborative relationships with referring agents and Primary Care Health Professionals.
