Mental Health Practitioner

As a Band 5 Mental Health Practitioner, you will provide a Mental Health service to an allocated caseload of Service Users who are under the STEPS Community Enhanced Care Pathway. This newly developed pathway offers more intensive multi-disciplinary community support to a specific group of Service Users. Your role will involve delivering a set of defined interventions within that caseload, with responsibility for the assessment of needs, planning and implementation of treatment and care to meet those needs, as agreed with the multidisciplinary team and/or supervisor.
Main Duties of the Job
- Liaise with other members of the multidisciplinary team and other agencies involved in the care of individual Service Users.
- Participate in Integrated Governance processes, particularly as they relate to your own practice, contributing to the monitoring of service provision and performance, and standards of care, through data collection and audit processes.
- Liaise with other services and agencies involved in the provision of services to people with Mental Health and other vulnerabilities.

Job Responsibilities
- Develop and maintain effective helping relationships with Service Users, working in partnership with Service Users and careers, and ensuring that your practice accords with guidance and best practice standards regarding the delivery of services.
- Provide screening and triage assessment for allocated Service Users under the Enhanced Care Pathway.
- Provide a defined Mental Health triage, assessment and interventions service to an allocated caseload of Service Users, having responsibility for the assessment of needs and the planning and implementation of treatment and care to meet those needs, based on evidence of best practice, and as agreed with the multidisciplinary team and/or supervisor.
- Work with Service Users and others as appropriate, in working collaboratively in the screening and ongoing assessment of risk, and the development and review of plans to manage identified risks effectively.
- Co-ordinate care for designated Service Users on your caseload under the supervision of a more senior practitioner.
- Develop and maintain good working relationships and effective communication with other members of your designated Mental Health or Specialised environment.
- Maintain accurate and up to date records of casework with individual Service Users, ensuring that your practice and record-keeping complies with all relevant standards and Good Practice Guidelines.
- Ensure the monitoring and evaluation of the Service by collecting and recording high quality and timely data using electronic and paper systems as required.
- Participate in your own supervision, working with your line manager to set and review your objectives, and to identify and meet your training and professional development needs.
- Ensure you keep up to date with developments in the Mental Health and Eating Disorders field and related areas, ensuring that your practice is based on best available evidence and meets all relevant standards and requirements set out in Integrated Governance frameworks.

Person Specification
Essential:
- Relevant education to degree level or ability to demonstrate equivalent level of knowledge and experience.
- Experience as a practitioner working with Service Users and careers, relevant to the delivery of Specialist Eating Disorder or Mental Health services.
- Able to develop therapeutically effective relationships with Service Users and careers, working effectively with diverse groups.
- Ability to form effective working relationships with other partners, patient groups and stakeholders.
- Organisational and time management skills.
- Ability to work within an ever-changing environment.
Desirable:
- Empathy for individuals who have experienced mental health problems and ability to cope with behaviour which may be challenging at times.
- Ability to use appropriate transport.
