We are seeking an experienced Mental Health Practitioner to join our dynamic team in South Bristol. This full-time role will involve working within a General Practice setting, providing expert care and support to patients in need.

The successful candidate will be part of a multidisciplinary team, collaborating with primary care professionals, voluntary services, and other stakeholders to deliver a personalized, preventative, and proactive integrated approach to mental health care. This innovative service design aims to address the complex needs of local populations, including issues such as housing, debt, carer fatigue, and addictions.

As a Mental Health Practitioner, you will work closely with patients, their families, and community members to ensure that mental health problems are addressed in conjunction with other critical issues. You will be an integral part of our one-team approach, working alongside care navigators, peer support workers, and other professionals to provide comprehensive care from GP surgeries.
