Job Title: Nurse Practitioner - Elderly Care Specialist

We are seeking an experienced Nurse Practitioner to join our team in a hospital setting, specializing in elderly care. As a key member of our multidisciplinary team, you will be responsible for ensuring the smooth transition of patients from hospital to community care. Your expertise in navigating discharge processes and care pathways will be invaluable in this role.

Key Responsibilities:

  • Conduct professional and timely patient assessments to identify barriers to discharge.
  • Work collaboratively with the multidisciplinary team to develop and implement effective discharge plans.
  • Ensure compliance with all relevant discharge processes and documentation.
  • Provide expert advice on care packages and discharge pathways to patients and their families.

Requirements:

  • Registered Nurse (RGN) with experience in a nursing home or hospital discharge settings.
  • Strong communication and liaison skills with multidisciplinary teams.
  • Expertise in navigating discharge processes and care pathways.
  • Ability to work independently as a neutral Discharge Assessor.
