Job Summary

About the Role

As a Professional Social Care Practitioner, you will be responsible for assessing the needs of citizens and carers, planning, implementing, reviewing, and evaluating outcomes. You will work collaboratively with individuals, taking into account their unique circumstances and priorities, to champion choice and control.

You will support people to make informed decisions about their care, empowering them to have services organized and risks managed in ways that suit them best. In addition, you will maintain professional standards, keeping up-to-date with changing needs and developing innovative solutions to meet the evolving needs of service users.

Requirements
  • Registered with Social Work England, with a Social Work or relevant SWE qualification
  • Ability to adapt interventions in response to diverse individuals and working environments
  • Natural collaborative approach, with flair for building relationships and working in partnership
  • Proven talent for involving people in decision-making and building on their strengths
