Dementia Care Coordinator
Salary:£30,000 - £35,000 per annum, dependent on experience.
About the Role:This is an exciting opportunity to join our team as a Dementia Care Coordinator. The successful candidate will work closely with service users and their supporters to provide high-quality care and support. You will have a strong background in mental health nursing, with at least 3 years' post-graduate experience.
You will be responsible for assessing, planning, implementing, and evaluating treatment and collaborative care planning for service users. You will also work collaboratively with colleagues to coordinate the duties and responsibilities of other nursing staff within the service.
Key Responsibilities:- To assess, plan, implement, and evaluate treatment and collaborative care planning for service users.
- To work collaboratively with colleagues to coordinate the duties and responsibilities of other nursing staff within the service.
- To ensure that each allocated service user has a comprehensive, clear, and up-to-date collaborative care plan.
- Registered Nurse Mental Health, 3 years post-graduate.
- Working knowledge and understanding of older adults with organic mental health.
