Main Responsibilities:- Provide expert care to patients with palliative care needs, working closely with the multidisciplinary team to ensure comprehensive support.
- Develop and implement individualized care plans, taking into account the patient's physical, emotional, and social needs.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Conduct clinical assessments, identify areas for improvement, and implement evidence-based practices to enhance patient outcomes.
- Provide education and support to patients, families, and carers, promoting self-management and empowerment.
- Registered General Nurse (RGN) with a post-registration qualification in palliative care or a related field.
- First degree in a relevant subject, such as nursing or a related field.
- Experience in caring for patients with palliative care needs, preferably in a community or hospital setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a flexible approach to meet the needs of the service.
