About the Role

We are seeking a highly skilled Community Care Specialist to join our team in Wokingham Borough Council. As a key member of our Adult Social Care Team, you will be responsible for conducting assessments and reviews, developing support plans, and working collaboratively with individuals, their carers, and other stakeholders.

As a Community Care Specialist, you will work closely with vulnerable adults, identifying their needs and delivering tailored support to promote independence, well-being, and safety. You will have a strong understanding of relevant legal frameworks, policies, and guidance, ensuring that your practice is informed and effective.

Key Responsibilities:

  • To conduct comprehensive assessments and reviews of vulnerable adults' needs, identifying areas for support and intervention.
  • To develop and implement personalized support plans, promoting independence, well-being, and safety.
  • To work collaboratively with individuals, their carers, families, and other professionals to gather information, share knowledge, and coordinate services.
  • To maintain accurate records, reporting on progress, and evaluating the effectiveness of support plans.

Requirements:

  • Degree or equivalent in a relevant field (e.g., social work, psychology, or related discipline).
  • Minimum 1 year post-qualification experience in adult social care, with a focus on supporting vulnerable adults.
  • Demonstrable expertise in risk management, communication, and interpersonal skills.
  • Full driving license and access to own vehicle (for travel between locations).

About the Salary and Benefits:

The salary range for this role is £41,511 - £47,754 per annum, plus benefits including annual leave entitlement, pension scheme enrollment, employee assistance program, local & lifestyle discounts, and use of a free onsite gym.
