**Job Title:** Care Support Specialist

About the Role

We are seeking a skilled Care Support Specialist to join our team in Slough. This is an exciting opportunity for someone who wants to make a difference in the lives of vulnerable individuals with mental health needs.

Key Responsibilities:

  • Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
  • Conduct assessments of needs and potential risks, agreeing levels of support and actions
  • Create support/action plans for providing appropriate services based on the assessment and reflecting available services and resources
  • Support customers to undertake domestic tasks, including practical assistance where necessary, to ensure high-quality living standards
  • Assist individuals in managing prescribed medication
  • Develop and maintain relationships with local community agencies and service providers
  • Empower customers to access entitled services and benefits
  • Carry out health and safety checks in individual rooms and communal areas
  • Identify and report safeguarding concerns appropriately
  • Promptly report and follow up maintenance issues
  • Manage administrative tasks and maintain accurate tenant/customer records
  • Encourage timely rent payments and effective management of rent accounts with the Housing Management team
  • Proactively chase referrals to efficiently utilize available properties
  • Adhere to Look Ahead's Policies and Procedures
  • Participate in learning and development activities to enhance knowledge and skills

Requirements

  • NVQ Level 2/3 or equivalent, or experience in the social care/charity sector
  • Practical understanding of mental health conditions and their impact on affected individuals
  • Excellent communication skills, including spoken and written English in both formal and informal contexts
  • Demonstrable ability to build positive professional relationships
  • Experience working as part of a team
  • Proficiency in MS Office and ability to use computers and learn new systems
