Community Social Care Specialist

**Job Overview**

We are looking for a skilled Community Social Care Specialist to join our Adults Community Mental Health Team in Slough. As part of a dynamic, multi-disciplinary team, you will be responsible for assessment, care planning, and service purchasing.
