Care Pathway Coordinator

We are proud to offer an exciting opportunity for a Senior Mental Health Practitioner to join our team at North Staffordshire Combined Healthcare Trust. As a key member of our ISH Team, you will contribute to the development of new care pathways that provide intensive support and interventions to children, young people, and their families.
As a Senior Mental Health Practitioner, you will be responsible for delivering care and supervising others who carry out all aspects of the caring process. You will also assist in setting and maintaining standards of care, participate in managing the team and unit effectively, and ensure integration with the multi-disciplinary team.
This role requires post-registration experience to post-graduate diploma or Master's level. A Disclosure and Barring Service check will be necessary for this position.
Key Responsibilities- Act in accordance with the NMC code of professional conduct and perform all aspects of the role in accordance with Trust policies and other national bodies.
- Participate in assessing care needs, developing, implementing, and evaluating individual care plans.
- Liaise with the multi-disciplinary team and outside agencies to implement patient care.
The estimated salary for this role is £45,000 - £60,000 per annum, based on experience.
