SEND Care Coordinator

Key Responsibilities:
- Co-ordinate and facilitate assessments of need, including the development of education, health & care plans that align with relevant legislation and the Code of Practice.
- Work closely with individuals, their families, and a range of professionals to ensure comprehensive support and services are provided.
- Develop and implement tailored strategies to address individual needs and promote independence.
Requirements
To succeed in this role, you will need:
- Proven experience in working with individuals with SEND and their families, with a focus on promoting independence and choice.
- Strong analytical skills, with the ability to interpret complex information and develop effective solutions.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and professionals.
