Care Coordinator for Cancer Patients

The University Hospitals of North Midlands NHS Trust is seeking a skilled Nurse to fill the position of Care Coordinator for Cancer Patients. About the Role:In this key role, you will be responsible for coordinating care services, ensuring seamless transitions between hospital and community settings. You will work closely with multidisciplinary teams to develop and implement effective treatment plans, promoting holistic care that addresses the physical, emotional, and psychological needs of individuals affected by cancer.
Responsibilities:- Coordinate care services, liaising with healthcare professionals, patients, and carers to ensure continuity of care
- Develop and maintain accurate patient records, communicating effectively with stakeholders to ensure timely and effective decision-making
- Contribute to quality improvement initiatives, identifying areas for enhancement and implementing changes to improve patient care and outcomes
