Job Title: Community Therapy Support Specialist
Description:
We are looking for an enthusiastic and highly motivated therapy rehabilitation assistant to join our Hertfordshire Community Team in Hemel Hempstead and surrounding area.
The role involves working with patients in their own home to provide rehabilitation services and promote independence. Our multidisciplinary team comprises of physiotherapists, occupational therapists, nurses, healthcare and therapy assistants, as well as support from administrators.
We work collaboratively with GPs, acute hospitals, social care, hospices and other third sector organizations. We have a well-established structure for clinical supervision and opportunities for practice development including in-service training and skills courses.
This is a part-time post based in the Dacorum Community Team in Hertfordshire. Estimated salary: £25,000 - £30,000 per annum depending on experience.
Key Responsibilities:
- To be an active member of the clinical rehabilitation team.
- Provide individual therapy treatment under the guidance of physiotherapy and occupational therapy colleagues, in the community setting.
- Build and maintain relationships with other local services to facilitate appropriate referrals and high-quality healthcare.
- Provide guidance and information to patients, health and social care professionals, carers and relatives and other non-professionals.
- Deliver appropriate treatment in a variety of settings including the patient's own home, residential and nursing homes and day centres.
