About the Role

We are seeking a highly motivated and empathetic Support Worker to join our team at Excelcare Holdings. As a Support Worker, you will play a vital role in empowering and guiding our residents to live as independently as possible.

Key Responsibilities:

  • Provide a comprehensive support service to residents, working on a strengths-based approach to meet their practical and emotional needs.
  • Encourage residents to develop and rediscover social skills, promoting their independence and well-being.
  • Support residents in developing activities that encourage daily living skills, such as personal care and household management.
  • Assist with general financial management, ensuring residents have access to the resources they need.
  • Ensure residents recognize their mental health needs and comply with prescribed medications.
  • Endorse good health and well-being, promoting the benefits of adhering to general practices.
  • Update support plans and record changes, incidents, and accidents, ensuring resident safety and well-being.
  • Create SMART support plans and comprehensive risk assessments to empower residents and prevent harm.
About You

We are looking for a candidate who is a strong communicator, able to work on their own initiative as well as part of a team. You will be patient, a good listener, and empathetic, with a professional and respectful approach to colleagues and residents.
