Job Description

We are seeking a highly skilled and compassionate Supported Pathway Officer to join our team at Brackenberry Ltd. As a key member of our team, you will play a vital role in supporting individuals experiencing homelessness with complex needs.

Key Responsibilities
  • Provide High-Quality Customer Service: Deliver exceptional customer service to clients, ensuring they are treated with respect and courtesy, and meeting all council standards and targets related to communication.
  • Support Residents to Reach Goals: Assist and motivate residents to achieve set goals and progress along a clear pathway for housing, providing client-centered support plans and intensive assertive keyworking support.
  • Coordinate Support Services: Coordinate necessary support services to assess residents' ability to live independently, identify services that can assist them in sustaining their wellbeing and tenancy, and provide a time-limited floating support service to ensure residents are linked into appropriate services after moving out.
  • Encourage Engagement and Participation: Encourage residents to engage and participate in activities that address issues of social isolation and confidence building, and accompany them where necessary and appropriate.
  • Advocate for Residents: Advocate for residents where required, providing practical support if needed to ensure their rooms are habitable and safe, and accompanying them to appointments as necessary.
Requirements
  • Knowledge and Experience: Possess knowledge of the impact of Adverse Child Experiences and the principles of Trauma-Informed Care when dealing with homeless people with complex needs, as well as experience of working with homeless individuals with complex support needs.
  • Qualifications and Skills: Hold relevant qualifications and possess skills in recovery-focused and harm minimization service provision, risk assessment, keyworking, and support planning.
  • Health and Safety: Be aware of health and safety issues in a supported housing setting.
