**Job description:**

We are seeking a dedicated Community Health and Wellbeing Practitioner to join our team in Sheffield. As a Band 6 Nurse, you will work as part of a duty system, assessing and planning care for service users. Your responsibilities will include delivering supervision to more junior members of staff and contributing to multidisciplinary service development.

**Key duties and responsibilities:**

  • Acting as a Named Worker
  • Comprehensive assessment of new referrals
  • Carrying a varied caseload defined by need
  • Contributing to a duty system on a rota basis
  • Delivering supervision to more junior members of staff
  • Contribute to Multidisciplinary service development

**Clinical responsibilities and requirements:**

  • Triage and determine urgency of new referrals to the service
  • Carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree the most appropriate course of action
  • To assess, evaluate and manage clinical risk
  • To formulate a plan of care based on identified need
  • Deliver evidence-based interventions as appropriate
  • Review effectiveness of care plan at regular intervals
  • To work within safeguarding processes, including where required to act as safeguarding investigator / manager
  • To work collaboratively with service users, carers and partner agencies
  • To maintain computer-held care records in an accurate and timely fashion
  • Undertake all work in a culturally sensitive manner
  • Act in accordance with Care Management System, Acute and Schedule Care Pathways as appropriate
  • Develop effective working relationships with Primary Care colleagues

**Salary range:** £41,000 - £47,000 per annum
