Medication Assistance Program Coordinator LPN

Job Summary
The Commonwealth of Virginia is seeking a highly motivated LPN to join a team of professionals committed to providing public health services. This position serves as the Medication Assistance Program Coordinator and provides assistance to medically indigent individuals in obtaining prescription and non-prescription items.
Responsibilities
- Interview clients, provide counseling according to applicable program policies, procedures, and health guidelines.
- Apply critical thinking and cultural awareness to all communication modes.
- Complete required documentation in client chart.
- Make appropriate referrals for medical care and community resources.
- Provide administrative assistance as needed to support HD activities and programs.
- May perform other duties as assigned.
- May be required to assist in the event of an emergency declaration.
Requirements
- Licensure as Practical Nurse required by state code.
- Knowledge of techniques and practices of the nursing profession, of public health practices, of quality assurance concepts, of teaching principles, of principles of decision making and problem solving techniques, of accurate record keeping and quality control, of socio-economic conditions relating to personal needs of clients
- Skills in performing technical procedures, in communication orally and written, in interacting with clients and the general public
- Ability to document clearly and accurately utilizing the health department's accepted form of documentation
- Ability to establish and maintain working relationships with individuals and families to function as a team member
Additional Considerations
- Current CPR certification.
- Prior experience in a public health setting preferred.
