Children's Community Specialist Nurse

We are seeking a highly skilled and experienced Paediatric Nurse to join our Children's Community Specialist Nursing (CCN) Team. As a key member of our team, you will be responsible for delivering holistic nursing care to children and young people with a range of health needs, ensuring their optimal health outcomes.
Main Responsibilities- Provide skilled paediatric nursing care to children and young people within their own home, or other applicable venues, to support families in the care of their children at home.
- Ensure continuity of care is established and appropriate support is available for children, young people, and families, with the aim of preventing unnecessary admission or attendance at hospital and facilitating earlier discharge where appropriate.
- Act as a named nurse for a caseload of generic children and young people, responsible for planning and co-ordinating care, delivering, and evaluating the care given.
- Complete accurate client records in line with CPFT policy, maintaining excellent communication with the child and family on the caseload and as their advocate.

Requirements- RSCN/Child Branch qualification
- Teaching/Mentorship/Practice Assessor qualification
- Extensive and broad experience of sick children's/paediatric nursing
- Caseload management
- Mentoring skills
