Children's Community Specialist Nurse

We are seeking a highly skilled and experienced Paediatric Nurse to join our Children's Community Specialist Nursing (CCN) Team. As a key member of our team, you will be responsible for delivering holistic nursing care to children and young people with a range of health needs, optimising their health outcomes.
Main Responsibilities- Provide skilled paediatric nursing care to children and young people within their own home, or other applicable venues, to ensure families are supported in the care of their children at home.
- Ensure continuity of care is established and appropriate support available for children, young people, and families, with the aim of preventing unnecessary admission or attendance at hospital and facilitating earlier discharge where appropriate from both local and tertiary hospitals.
- Act as a named nurse for a caseload of generic children and young people, responsible for planning and co-ordinating care, delivering and evaluating the care given.

Requirements- RSCN / Child Branch qualification
- Teaching/Mentorship/Practice Assessor qualification
- Extensive and broad experience of sick children's/paediatric nursing
- Caseload management
- Mentoring skills
