Job Summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a highly skilled and dedicated Paediatric Community Specialist Nurse to join our team. As a Paediatric Community Specialist Nurse, you will be responsible for providing high-quality, patient-centered care to children and young people with complex health needs in their own homes or other community settings.

Main Responsibilities
  • Provide skilled paediatric nursing care to children and young people with complex health needs in their own homes or other community settings.
  • Develop and implement individualized care plans in collaboration with the child, family, and multidisciplinary team.
  • Conduct comprehensive assessments and evaluations to identify the child's needs and develop strategies to address them.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Provide education and support to families and caregivers to promote healthy habits and self-management skills.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Registered Children's Nurse (RSCN) or equivalent qualification.
  • Experience in paediatric community nursing or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work in a fast-paced environment and adapt to changing priorities.
