About the Role

We are seeking a highly motivated and experienced Community Mental Health Practitioner to join our dynamic team at Devon Partnership NHS Trust. As a key member of our Honiton Community Mental Health Team, you will play a vital role in delivering high-quality care to individuals with mental health needs.

Main Responsibilities

  • Assess and care plan for clients with a range of mental health problems
  • Work collaboratively with multidisciplinary teams to deliver comprehensive care
  • Develop and deliver psychological group interventions
  • Contribute to the development of physical health screening and monitoring

Requirements

  • Postgraduate experience in mental health and/or learning disabilities
  • Excellent communication and assessment skills
  • Ability to exercise clinical judgement in risk assessment
  • Commitment to recovery principles and values
