About the Role

We are seeking a motivated and enthusiastic Physiotherapist to join our Honiton, Ottery St Mary and Cranbrook Community Rehab Team in a full-time substantive post. As a key member of our team, you will be working in partnership with health and social care colleagues to provide integrated care services across various settings.

As a Physiotherapist, you will be responsible for undertaking comprehensive assessments and interventions to support adults of all ages in promoting independence, healthy living, and rehabilitation. You will work alongside a multidisciplinary team, including Physiotherapists, Occupational Therapists, a Rehab Nurse, and Rehab Support Workers.

Key Responsibilities
  • Undertake comprehensive assessments and interventions to support adults of all ages in promoting independence, healthy living, and rehabilitation.
  • Work alongside a multidisciplinary team to provide integrated care services across various settings.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with health and social care colleagues to ensure seamless care transitions.
Requirements

We are looking for a qualified Physiotherapist with a passion for rehabilitation and a commitment to providing high-quality patient care. You will have excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.

You will be required to work within the Urgent Community Response Team to support with delivery of the Ageing Well rota, working shifts over seven days a week and between the hours of 08:00 to 20:00 on a rota basis.
