About the Role

We are seeking a highly motivated and experienced Support Worker to join our team at Cartrefi Cymru Co-operative. As a key member of our team, you will play a vital role in delivering high-quality support and care to individuals with learning disabilities, mental health needs, older people, people with Autism, and physical disabilities across Wales.

Key Responsibilities
  • Provide person-centered support to individuals, promoting their independence and well-being.
  • Work closely with Area Managers to develop and implement support plans, ensuring the best possible outcomes for our service users.
  • Build and maintain strong relationships with service users, their families, and other stakeholders.
  • Contribute to the development and delivery of community-based activities and programs, enhancing the lives of our service users.
  • Collaborate with other professionals to ensure seamless support and care.

Requirements
  • A commitment to enabling people to live fulfilled and independent lives.
  • A full valid driving license and a car, which you are able to use for work purposes, as driving is essential.
  • A Level 3/5 qualification in Health & Social Care.
  • Good organisational skills and the ability to prioritise your workload and meet deadlines.
  • Good communication and networking skills, with the ability to deal with people at all levels.
  • Able to think creatively and build on opportunities.
  • Computer literate and have a thorough knowledge and experience of working with Microsoft Office packages, including Word, Excel, and Outlook.
  • Fully flexible over 7 days, as evening and weekends will be required.
