About the Role

The Midlands Partnership NHS Foundation Trust is seeking a skilled and compassionate Community Mental Health Nurse to join our team in Telford. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals experiencing mental health crises.

Key Responsibilities

  • Assess and evaluate mental health crises, delivering safe and effective home treatment in accordance with individual wellbeing and recovery plans.
  • Work collaboratively with internal and external agencies to provide specialist assessment and care planning.
  • Triage and assess referrals, coordinating the work of the team to ensure seamless care delivery.
  • Develop and maintain strong relationships with service users, carers, and colleagues to promote a culture of inclusivity and respect.

Person Specification

  • Registered Mental Health Nurse, active on the Nursing and Midwifery Council register.
  • Previous experience working in the community, with a strong understanding of mental health principles and practices.
