We are seeking a highly skilled and experienced District Nurse Team Leader to join our innovative and dedicated team at Oxford Health NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering outstanding care to the communities of Oxfordshire.
Main Responsibilities- Lead a team of community nurses in providing high-quality, evidence-based care to patients in their own homes.
- Develop and implement effective care plans, working closely with patients, families, and other healthcare professionals.
- Collaborate with other teams and services to ensure seamless care transitions and optimal patient outcomes.
- Contribute to the development and implementation of community nursing strategies, policies, and procedures.
- Provide clinical supervision and mentorship to junior staff members.
- Registered General Nurse (RGN) with current NMC registration.
- Level 6 study or higher.
- Formal management and/or leadership experience within healthcare or extensive clinical experience as a Registered Nurse within a District Nursing team.
- Community nurse experience and knowledge of community nursing practices.
- Excellent communication, interpersonal, and leadership skills.
- Ability to work autonomously and as part of a team.
