Job Summary

The Kirklees Council is seeking a skilled and experienced professional to fill the role of Trusted Assessor. This position involves working collaboratively with partners across the social care and health system to deliver effective timely discharges to Care Homes across Kirklees, Calderdale, and Wakefield.

Key Responsibilities

  • Collaborate with care home managers to gather information for assessments
  • Work with healthcare professionals to ensure seamless transitions from hospital to care settings
  • Develop and maintain relationships with care providers to facilitate efficient discharges
  • Contribute to the development of strategies to reduce waiting times and improve patient outcomes

Requirements

  • Relevant experience working in the social care and health sector
  • Strong understanding of the issues faced by the care sector
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a broader team
