Job Title: Trusted Assessor

Job Type: Temporary, Part-time

Location: Huddersfield Royal Infirmary, with some travel to other hospitals

Hours: 30 hours per week, Tuesday to Friday

Job Summary:

We are seeking a skilled and experienced Trusted Assessor to join our team at Kirklees Council. As a Trusted Assessor, you will play a critical role in supporting the discharge system in hospitals across Kirklees, Calderdale, and Wakefield.

Key Responsibilities:

  • Collaborate with healthcare professionals and social care teams to assess patients' needs and develop discharge plans
  • Work with care homes and other healthcare providers to ensure smooth transitions and reduce waiting times
  • Provide expert advice and guidance to healthcare professionals on discharge planning and patient care
  • Contribute to the development and implementation of strategies to improve discharge processes and patient outcomes

Requirements:

  • Relevant experience in healthcare or social care, with a strong understanding of discharge planning and patient care
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with healthcare professionals and social care teams
  • Strong analytical and problem-solving skills, with the ability to assess complex patient needs and develop effective discharge plans
  • Ability to work flexibly and adapt to changing priorities and deadlines
